Wine and Sleep After 40: What Two Glasses Actually Does

You are exhausted at 8pm and wide awake from midnight to 6am. Seven hours, according to the ring on your finger. You wake up feeling like you have been hit by a truck.

And you already know why, because you have a list. It is the kids. It is your hormones. It is the stretch you have been in since spring. It is that your friend runs the same week you do and does not seem to feel any of this.

The glass of wine you poured while you unloaded the dishwasher is not on that list. You are not ruling it out, but it's not like you're drinking a bottle out with your girlfriends like you did in your 20s.

Here is the short version. Alcohol cuts into REM sleep at a much lower dose than most women are told. Around two standard drinks is already enough to delay REM and shorten it, and it gets worse from there. What alcohol does not reliably change is how many hours you sleep. So the hours look fine, your tracker agrees with you, and the part of sleep that changed is not the part anyone measures.

That is why you could never prove it.

Why you could never prove it was the wine

The usual story about alcohol and sleep comes with a picture attached. You drink, you go under, you snap awake at 3am, you blame the wine. It is tidy. It is also the part the evidence is least sure about.

A 2024 systematic review in Sleep Medicine Reviews pooled 27 studies on what alcohol does to sleep in healthy adults (Gardiner et al.). The authors could not pin down a reliable effect on total sleep time, on sleep efficiency, or on how much you wake in the night. The uncertainty there was large.

What they could pin down was consistent. Alcohol delays the start of REM sleep and reduces how much of it you get.

Put those two findings next to each other and you have the whole problem. Alcohol does not shorten the night. It changes what the night is made of. You still spend seven hours asleep, and a smaller share of those seven hours is REM.

Nothing you own will show you that. A ring gives you an estimate built from movement and heart rate, not a reading of your brain. The only number you are ever handed is the total, and the total is the one number that holds steady.

If that pattern feels familiar, it should. It is the same shape as being told your labs are normal while you feel awful. A normal-looking total does not mean nothing is wrong.

Why losing REM shows up in your day

It is fair to ask why one stage of sleep should matter this much when you are still getting the hours.

REM is when the brain does its work on emotional material. A 2017 review in Sleep Medicine Reviews gathered the evidence that sleep supports emotional memory through every stage of processing, and that losing sleep degrades both how you take emotional information in and how you file it (Tempesta et al.). The same review describes sleep's role in what it calls adaptive next-day emotional reactivity, which is the technical way of asking whether small irritations stay small the following afternoon. Emotionally loaded material, the authors note, appears to depend particularly on REM.

A broader review of sleep physiology states the cost more plainly. Disrupting the restorative work of sleep produces changes in emotional reactivity along with impairments in attention, memory, and decision making (Brown et al., Physiological Reviews).

Read that list again. Attention. Memory. Decision making. You know those three as your Wednesday: the word that leaves your head in the middle of a sentence, the email you read four times, standing in the pantry at 5:30 with no idea what to feed anyone.

So when the pour costs you REM, the loss is not abstract. It is the exact capacity you spend your whole day drawing on.

Two standard drinks is already the dose

The same review found a dose-response relationship, and the threshold sits lower than almost anyone expects.

At roughly two standard drinks, REM is already delayed and already shorter. That is the low end of what these studies tested, and the disruption climbs steadily with every drink above it.

Two standard drinks is a generous pour and a top-up. It is a Tuesday.

A standard drink is five ounces of wine, which is smaller than most glasses poured at home.

Why the nightcap stopped working

There is an asymmetry buried in that paper that nobody gets told about.

Falling asleep faster is the entire reason the nightcap exists. In the pooled data, that effect only appeared at the high end, somewhere around five drinks. And the authors note that when it does appear, it probably makes the REM disruption that follows worse.

Put the two findings side by side and the nightly glass lands in the worst possible spot on the curve. It is plenty to take the REM hit. It is nowhere near enough to buy you the sleep you are drinking it for.

The nightcap is not doing the job it was hired for. That is a fact about a dose, not a comment on you.

What changed about the same glass at 45

Two shifts happen at once in your forties, and they stack.

The first is that sleep gets more breakable on its own. Hot flashes and night sweats interrupt it, and the review literature on insomnia in menopausal women lists alcohol among the common contributors to disrupted sleep in this group (Bonanni et al., Maturitas 2019). One finding in that same work deserves more attention than it gets: disrupted sleep predicts mood disturbance more strongly than the hot flashes themselves do. The broken sleep is doing more damage than the symptom everyone talks about. It is also the same window in which focus starts slipping, which is rarely treated as one problem even though it usually is.

The second is that a 2024 narrative review of alcohol use at midlife describes the same overlap from the other direction, with alcohol touching hot flashes, sleep, and mood together (Shihab et al., Maturitas 2024).

Now the honest caveat, because we would rather hand you the truth than a cleaner story. That midlife literature is built largely around heavier drinking. It does not settle what one glass does to one woman at 47. What it does establish is that the same amount of alcohol is now landing on a system carrying less margin than it had at 32.

How much less, for you specifically, the research has not caught up to yet.

Nobody ran this study on you

The 2024 review closes on a line that should be better known. The authors call for future research into whether alcohol affects sleep differently in women than in men.

Which means the question you are asking, what does my glass do to my sleep, in my body, at my age, has largely not been studied.

You have not been failing to find the answer. The answer has not been made yet.

Your labs are normal. Your sleep was seven hours. Your bloodwork came back fine. Every number you have been handed describes a population, and not one of them was built to describe you.

So should you just stop for two weeks?

You can, and plenty of women do. Some of them feel a real difference by the second week.

We should tell you what we see when it does not work, though, because it happens often enough to matter. She takes the wine out. She sleeps the same, wakes up the same, feels the same. She decides the wine was never the problem, that nothing works on her anyway, and she puts it back. She usually mentions it in passing, at the end of a call, while we are already wrapping up.

What happened is that the wine was one input into a night that had three or four problems in it. Pulling out one of four does not give you a clean answer. It gives you a muddy one. And a muddy answer at week two is how a capable woman talks herself into believing she is the exception.

What we look at before you give anything up

Before we would ask you to change anything about your evening, we want to see the rest of the night.

Thyroid, read against ranges tight enough to flag what a standard panel waves through. Blood sugar, because a dip in the back half of the night will pull a body awake on its own. Ferritin. And where you are in the perimenopausal transition, which is not one number and cannot be read off a single spot check, whatever the label you were handed says.

Then the wine becomes one variable inside a picture, instead of the only suspect in the room.

We are not going to tell you to give it up. We are going to tell you what two drinks does to REM, and what your own numbers say about how much margin you are working with, and then you get to make a real decision about your own Tuesday night.

That is a different conversation than the one you have been having with the internet at 11pm.
